Appointments
Same day appointments and walk-ins are welcome. Medical providers
are available to established patients for emergency telephone consultation
after hours and weekends. When registering as a new patient, please
arrive 30 minutes early and bring insurance cards, Medicaid or Medicare
cards, or proof of income documents.
Payments
& Insurance
Medicare, Medicaid, and private insurance
plans are honored. Your claims will be filed by NHCHC. There is
no deductible for Medicare patients. If you have no insurance,
Medicaid, or Medicare, you may apply for our sliding fee discount
for services. To apply for this discount, a proof of income is required
in one of the following forms:
